About CodaFend

CodaFend is the operating brand of Cast Rock Innovation L.L.C. d/b/a Cast Net Technology, a technology company specializing in compliance-critical software for regulated industries.

Our work sits at the intersection of healthcare administration, regulatory compliance, and applied artificial intelligence. We build software for organizations where documentation accuracy, audit defensibility, and regulatory adherence are operational necessities — not afterthoughts.

CodaFend was founded by a team with direct, hands-on experience inside Medicare Advantage operations — including professionals who have worked on CMS RADV audit responses, risk adjustment data validation workflows, and compliance remediation programs at health plans. That operational background informs every design decision: we understand what coders, auditors, and compliance officers need from software because we have been those people.

Industries We Serve

Our work spans regulated environments where precision, defensibility, and timeliness are mission-critical.

Medicare Advantage & Risk Adjustment

Organizations managing CMS risk-adjustment programs face continuous documentation requirements and periodic RADV audit exposure. We build tools that help compliance and coding teams maintain defensible, audit-ready records.

RADV HCC validation →

Healthcare Administration

From physician groups to managed care organizations, healthcare administrators operate under overlapping federal and state regulatory frameworks. Our software brings structure and traceability to documentation-heavy workflows.

Chart review software →

Regulatory Compliance — Small & Mid-Size Business

Businesses with 5–200 employees face the same regulatory obligations as large enterprises but without dedicated compliance staff. We build products that deliver professional-grade regulatory intelligence to operators who need clarity, not complexity.

Multi-State & Multi-Jurisdiction Operations

Companies operating across state lines face fragmented, rapidly-changing regulatory requirements from dozens of agencies simultaneously. Our monitoring tools surface what's relevant, when it matters, for the specific jurisdictions where a business operates.

Our Products

CodaFend products for Medicare Advantage compliance and regulatory intelligence — built for organizations where accuracy is not optional.

How It Works

From chart upload to reviewer-ready report — entirely on your infrastructure.

STEP 01Chart Ingestion

Upload PDFs on your internal network. All document processing happens inside your infrastructure — nothing is transmitted externally. Supports batch ingestion for high-volume review cycles. Zero PHI egress →

STEP 02Automated MEAT Analysis

The platform evaluates each HCC against MEAT criteria — Monitoring, Evaluation, Assessment, Treatment. Evidence is pinned to the exact source sentence in the chart, not interpolated or summarized. MEAT extraction →

STEP 03Reviewer-Ready Output

A structured report flags each HCC as supported, unsupported, or uncertain — with citations to source sentences. Your coder or auditor makes the final determination. The platform assists; it does not replace clinical judgment. Audit readiness →

Our Approach

Three principles that govern every product we ship.

Precision Over Speed

Compliance software must be accurate. We build systems that verify claims against source documents, flag uncertainty explicitly, and never overstate confidence.

Human-in-the-Loop

Our software assists decision-making; it does not replace it. Every output is designed to be reviewed, questioned, and acted on by a qualified professional.

Built for Audit Defensibility

Everything we ship is designed with the assumption that it will be examined by a regulator. Traceability, documentation, and evidence chains are not features — they are requirements.

Frequently Asked Questions

Answers to the questions compliance and risk adjustment teams ask before engaging. View all FAQ →

Will we need to sign a BAA?
Yes. Because RafCite™ processes Protected Health Information, a Business Associate Agreement is required before deployment. We have a standard BAA ready for execution and can accommodate your organization's preferred template. BAA execution is part of the standard onboarding process.
How does on-premise deployment work? Do we need a dedicated server?
RafCite™ deploys via containerized components that run on hardware you control — either a dedicated on-premise server or a private cloud environment with no external egress. We provide deployment documentation and onboarding support. A GPU-capable server is recommended for production throughput, but we will work with your infrastructure team to identify the right configuration for your volume requirements.
How is this different from manual chart review or our existing coding staff?
RafCite™ is not a replacement for your coders — it is a pre-screening layer that dramatically reduces the time each chart requires. Manual MEAT review typically takes 3–4 hours per chart; RafCite™ produces a structured, citation-backed analysis in under 5 minutes. Your coding staff then reviews the flagged HCCs and makes the final determination. The result is higher throughput, more consistent documentation standards, and a defensible audit trail — without eliminating the clinical judgment that regulators expect.
Is the platform updated for CMS HCC V28?
Yes. RafCite™ natively supports both HCC V28 (the current model) and V24 (the legacy model still used for certain payment year blends). RAF weight calculations and hierarchy exclusion rules are applied automatically based on the payment year you specify. We track CMS updates and release model updates accordingly.
What does pricing look like?
RafCite™ is priced as an annual platform license — not a per-chart or per-seat consumption model. There are no cloud processing fees, no data egress charges, and no usage-based cost surprises at audit time. Pilot engagements are available for qualified organizations. Contact us to discuss pricing relative to your chart volume and review cycle.
What support is included?
All engagements include deployment support, onboarding documentation, and direct access to the technical team during initial rollout. Ongoing support terms are included in the license agreement. We also provide updates as CMS model specifications change — you will not be left running an outdated version at the start of a new payment year.
