What is RafCite?

RafCite is on-premise Medicare Advantage HCC chart review software that evaluates clinical documentation against MEAT criteria (Monitoring, Evaluation, Assessment, and Treatment) and produces audit-ready evidence packets. It processes medical records in under 5 minutes per chart with zero PHI egress.

Who is RafCite designed for?

RafCite is designed for Medicare Advantage health plans, risk adjustment vendors, compliance teams, and coding operations that need to validate HCC documentation at scale while keeping all Protected Health Information on-premise. What is RuleBrief?

RADV and HCC Validation

What is RADV?

RADV stands for Risk Adjustment Data Validation. It is the audit process CMS uses to verify that HCC (Hierarchical Condition Category) codes submitted by Medicare Advantage organizations are supported by adequate medical record documentation. How does RafCite support RADV audit readiness?

RafCite automates the HCC validation process by reading medical records, evaluating each HCC code against MEAT criteria, extracting exact source citations, and producing structured evidence packets. This gives compliance teams audit-ready documentation in under 5 minutes per chart—instead of the 3–4 hours typical of manual review.

What is MEAT criteria?

MEAT stands for Monitoring, Evaluation, Assessment, and Treatment. It is the CMS documentation standard that defines the types of clinical evidence required to support an HCC code. For an HCC to be valid under RADV audit, the medical record must contain documentation meeting at least one MEAT category. RafCite performs automated MEAT evidence extraction for every chart it processes.

What HCC model versions does RafCite support?

RafCite natively supports CMS HCC V28 (current model) and V24 (legacy model) used for certain payment year blends. RAF weight calculations and hierarchy exclusion rules are applied automatically based on the payment year you specify.

How fast is the validation process?

RafCite processes a typical chart in under 5 minutes, producing a complete evidence packet with MEAT citations and confidence indicators. Manual chart review typically takes 3–4 hours per chart. Processing speed scales with available compute resources, and a GPU-capable server is recommended for production throughput.

Deployment and Security

How does on-premise deployment work?

RafCite deploys as containerized components on hardware you control—either dedicated on-premise servers or a private cloud environment. Your infrastructure team manages the environment, and CodaFend provides deployment support and onboarding documentation. A GPU-capable server is recommended for production throughput.

What does zero PHI egress mean?

Zero PHI egress means no Protected Health Information leaves your network at any point during processing. RafCite does not call external cloud APIs, does not transmit data to third-party services, and does not require an internet connection for chart processing. Will we need a BAA with CodaFend?

Yes. A standard Business Associate Agreement (BAA) is available and executed as part of CodaFend's onboarding process. Contact legal@codafend.com for BAA requests and compliance-related inquiries.

What infrastructure is required to run RafCite?

A GPU-capable server is recommended for production throughput, though RafCite can run on CPU-only hardware at reduced speed. The software deploys via containers and works with existing infrastructure teams and standard server environments. CodaFend provides technical requirements documentation during the onboarding process.

Is data encrypted in RafCite?

Yes. Data is encrypted at rest and in transit within your network. RafCite follows healthcare data security best practices and is designed for environments where PHI protection is non-negotiable. All processing occurs within your controlled infrastructure with zero external data transmission.

What does RafCite pricing look like?

RafCite is licensed as an annual platform license. There are no per-chart fees, no per-seat charges, and no cloud processing costs.
